I have personally found that the hallucinations that accompany sleep deprivation, while kind of fun, tend to kind of negate any productivity.

That's something I don't get about med school. Why do they want us to work 36 hours straight? What patient wants you to treat them when you are seeing pretty little butterflies flying back and forth that aren't really there?!?

I don't know that it's possible to pass out in an "attractive" fashion... although I've been told after a few parties that I would have looked OK after passing out had I not been slobbering.

from reading posts and other info I think what I
want to do is keep all the good stuff and lose all the bad stuff...is
that possible?? and having the meds at night when I need to work or
continue venturing out into the social world will help that to

Political correctness is a joke...and not a very funny one at that. The very name, "political" and "correctness", is oxymoronic. When has politics ever been correct?
Shire had an all day ADHD hotline with specialists on-call to answer questions about the disorder and meds, etc. I called and asked the same question... is there anything that lasts all day? What can be done? I got a transcript of the conversation which includes the idea of adding a second dose of short acting meds. Sabina, I'd be happy to email it to you if it will help you persuade your doctor. Just let me know.
Oh, BTW, Shire (who else?) actually has a new version of Adderall in phase 3 trials with an anticipated release date sometime in 2006 (convenient, isn't it, when they're just about to lose the patent on Adderall XR? I hate the pharm industry...). It's an even longer acting version of the drug that right now is called spd465. They acknowledge that Adderall XR does not last long enough to get most through the day. I can't believe your doc is unfamiliar with this complaint! It's a very common problem.I'm just posting randomly now 'cause I don't wanna be a newbie anymore...

BTW, my doc and I compromised. His cap for me is 40mg/day, and he'll split that any way that works best for me. I used to take 40mg all at once in the AM, but when it wasn't lasting long enough we decided to try 30mg in the AM with a booster in the afternoon. That stretches it out for me without really losing any efficacy.

Well, it's not so much a holding
issue, more like diarhea: it just BLAAATZZZ out explosively messy at

My doc is fairly flexible. He trusts that I know my body and I know how I'm functioning. Using a booster dose is actually a relatively common phenomenon. Remember, these drugs were designed to get a child through school without visiting the nurse's office. The claim that they last all day is somewhat misleading. Have you tried doing some research on it yourself? Maybe if you could back up your request with some hard data your doc would be more receptive to the idea

Hi Kibbles & Shakespeare, thanks for the input, class is close to
what I have to do (constantly having to teach myself new things)
but hard for me as I'm working at home in the evenings and have to stay
on task...at work I can do that (usually) but at home...if the meds
wear off I find myself wandering around the apt. not working....and
going out socially, I went out and wasn't at my best and I said and did
some clumsy things, tired too but still..wish I could just say whatever
I wanted to (within bounds of course, no nasties...oh wait, I've done
that already), I feel like socially I'm afraid to say almost anything
anymore just to avoid seeing someone give me that look

I know the meds would help me to be more relaxed. My doc will have a
fit if I ask him for an extra dose but I am doing one today late
afternoon as I have two rogue 10mg Adderall and I need to work as many
hours as possible right now..Shakespeare, do you thing your Doc went
along cause you're a med student but wouldn't have otherwise?

are coming down from their meds...especially after feeling okay all day
and then some of the unwanted stuff resurfaces..take the meds later in
the day?
I had the same problem. I work and am a full time student in med school. I need sixteen hours coverage, at least, and my Adderall XR gave me about eight. My doc and I finally agreed to add 10mg of regular Adderall in the evening. It helps to stretch out the actions of the first dose, plus it gives me a little dosing flexibility. I can take an extra one if I need it or take it as late as I need to to meet the needs of my schedule.

I do that when I have classes at night at school.
I take mine later so that it will be wearing off about the end of the class. There's a trade off, however, you lose a little production time in the am 
It just depends what your goals are for the day, I guess.

I was recently switched from taking:
40-60 mg of Adderall p/day @ 20 mg per dose, occasionally I might do half of a tablet if I didn't need as much of the meds on board
to
40-60 mg @ 15 mg per dose and occasionally doing half a tablet when I am working in the evenings or don't need to focus for as long
This new regime has has been much better for me in terms my level of focus and my ability to hang with teaching exercise classes around 4 or 5 PM after having been up and running since 5 AM. I needed something that could sustain me through a pretty grueling schedule that undergoes frequent changes and where I travel from one place to another either teaching group exercise or doing personal training.
I also have a learning disorder. By taking a small dose about half hour to an hour before I teach, I've been able to completely annihalate the problems that I have which are associated with it:-))
The difference can be profound when you are dialed into a schedule/dosage that works. Whenever I forget to take the meds, I'm right back into the throes of dealing with my old learning disordered patterns of thinking and communicating.
If your doc isn't receptive to your situation, try another. My current medication schedule was prescribed by my second doc...the first was not very open to trying different things so I traded her in for someone else. I decided that I've suffered for 40 years with Add and the LD and I don't want to waste time working with a doc who doesn't take my best interests into consideration. There is too much positive out there waiting to happen by finding solutions that work. That's just my take on the matter though.
Taking half of a tablet has also helped to reduce some of the rebound effects I was experiencing on the other regime. I also use them in the am with a strategy I worked out that helps me to get up earlier and without being in a fog until around noon. My dosages vary b/c my work schedule is subject to such frequent changes. As a result, my doc has been flexibile in terms of how I adjust my dosages. This has worked out very well for me.

Just saw your post. I can relate to the PMS and "blonking", especially after the meds wear off!
BTW, if we're talking about the same thing, we call it "bonking" in Albania. Is it "blonking" in Canada...or is that the PMS kicking in (HA HA)?!

I'm brand new to the world of Adderall. I've found the different dosage variations mentioned on this board very insightful. My doctor rx'd me Adderall-xr20mg after our first visit (3 wks ago), and scheduled a follow-up with her 2 weeks later. I work in the medical field during the nights, (6pm-6am) and I found that the affects did not last much longer than 8 hours for me at 20mg. I just had my 2 week follow-up this past Friday, and I explained to her that this dosage wasn't lasting through-out my 12 hour shift, but worked very well. She bumped my dose up to 25mg-xr and it seems to last a little longer, but not much. Adderall has really improved my ability to function in every possible way. I'm on my third week with Adderall now, and it really does feel like I've missed out on the past 29 years of my life!
