I am only on 5 mg twice a day and can hardly tell I am on it at all. My doc wants me to come back in two weeks and I suppose he will increase it. I have not had the increased pain problem like you have had but I am on a very lose dose. You need to make an appointment with the doc or talk to his nurse instead of the secretary. If you make an appointment he has to see you. Dr.s are notorious for not calling you back but I would let him know how many times I called in case he was not getting the messages. What do you mean by your metablism is going faster? I hope you get the anwers you need and I understand it is important to vent. 
When i was on it, would get headaches anytime I quit for a few days, and anytime I started back. After about 1 to 2 weeks these taperred off.
Don't know about the pain threshold, for I was always like that since my nech injury.
I was taking 80 mg a day, 40 twice a day, and my heart rate stayed high (as in sitting at rest would be 105 bpm), I was always hot, and constantly hungry, but could focus. Yes we tried other things, and smaller doses, didn't work.
Got off meds once I got out of college, basicly to save my heart since heart problems run in my family.

Started on Ritalin this saturday..3 x 10 mg a day.
I am having lots of side effects.
I feel tired after the second pill, get headaches, I feel floaty, my heartrate goes up by 20 beats or so after the least bit of strain (like coughing even), I feel warm all day, my metabolism seems to go faster, and my pain-treshhold has gone up.
Yesterday I scratched my arm and it was a bit too hard I believe because now I have 2 6 inch scratches on my arm...So nice...At least they didn't bleed but it's like I was on ecstasy again, the feeling you get after 2 days partying...and I don't like it.
Phoned my MH doc (who prescribed the Ritalin in the first place) about it, but only got his secretary (as usual). Hope the b*stard calls back this time because I phoned him 3 times last week and no response...And he said he was there for me, or at least within 24 hrs..
I need his report on my ADHD for a programme for work (reintegration stuff) and I need it NOW.
Are all doctors like this? Unreachable when you need them???
