Good link for Teen Add questionnaire?

Does anyone have a good link for an online questionnaire I could have my son 14 yr old son take ?

We have made an appointment to have him evaluated and hes not too thrilled about it.

He has an Autistic brother and I think he is really fearing being labled.

I think if he can see questions and how many relate to him he may start to accept it and welcome some intervention.

I did check out the link you told me about and he definitely would come out as ADD.

I will gently break this to him and have him take it himself so that he can come to grips with it.

Once he understands it is an issue , I think it will actually help him realize that some of the issues he is having are beyond his immediate control.

Probably the best questionnaire for an older child or an adult can be found on Dr. Amen's website.  His treatment and diagnosing of ADHD is controversial, but he has an extensive self-test one can take.  You may want to checkout his website.
