More medicine helps them sleep?

My son is 7 and on Focalin XR 20 mg.  We give it to him in the morning, then when he gets home from school I give him 10 mg's of generic brand Ritalin (not time released).  When that where's off before bed he gets a boost of energy and very hyper.  His psychologist told me to give him more Ritalin because it will help him get to sleep.  Has anyone else been told this or does this work for anyone else?Have never heard this, but what do I know? My son is 8 and takes 10mg of Focalin XR in the morning, which wears off so gradually that I don't really notice anything. I've heard of melatonin or even benedryl for sleep, but never more ritalin.Jennew

My doctor specializes in ADD and said the same. I am finally at the point where I could fall asleep while on a smaller dose of Adderall but I couldn't do so earlier in my treatment (I've been on meds since last December).

So evidently it can work, however, I suspect that it doesn't work for all. Let us know the results.

When our Dr. said the same thing...I thought it sounded crazy.

So we raised her dose of Focalin XR. It worked!  On rare occasion we use melatonin. It helps her to relax and fall asleep faster. I usally give it to her an hour before bed. I tried them and they are very good and no drowiness in the am. You can also purchase in liquid.

Some children do seem "hyper" when their meds wear off. This can be rebounding. Switching to the patch as erased all rebounding so far.

Thank you for the feed-back.  I was also wondering if anyone who has tried this approach has noticed thier child or themselves waking up very early in the morning and being overly hyper then also, till the med's kick-in.

The wind up described by Jennew is a lot like what I experience my self. I do a lot better if I go to bed before the medication wears off, once it wears off I begin to wind up so I can't sleep. I have tried to let my self naturally wind down . . . .I was still up when the sun came up the next day.  . . . .natural wind down didn't work worth a poop.

ADD medications work to correct the dopamine problem in the brain. This is why they have a calming effect on hyperactive children. It make sense that the medications could help sleep regulation because the chemical imbalance is still being corrected, once the medications wear off the chemical imbalance returns and for some it returns with a vengeance.  

I know of some one who also takes a full dose of their ADD medications at night to help them sleep, and they report it works better than medication designed for sleep. It makes sense to me because non-ADDers must have the same amount of dopamine available for their frontal lobes even during the night other wise non-ADDers would become ADDers every night.

I have been on Adderall for nine years, and I do not experience any lack of REM sleep. I have taken my medication and promptly laid down and took a nap and I woke up feeling well rested.

 

 


Sorry to hear of your problems.  My 8 yr old son is on Focalin 20xr and 5mg two times a day. Thats alot.  I have been giving him Melatonin for quite some time and it ALWAYS works to get him to sleep at night.  If I gave him more stims, it would keep him up all night.  Melatonin is much safer than prescription sleep aids.  Maybe you could give that a try 1 hour before bedtime and a regular routine of calming time before bed such as reeading together or laying in bed together talking.  No TV or gameboy right before bed!

I know of several people whos kids, like mine, wake up extremely hyper til the meds kick in.  I believe its stim related. I havent found any answers for that one yet, but I wish you the best and let me know if you find answers.

Joshsammom, I was wondering what times you give your son the two 5 mg's?  Also, once he's asleep how does he sleep through the night and how much sleep each night do you think he gets?

The last 2 days I've been giving my son 5 mg's Focalin at 4:00 and it wears off before bed but it hasn't been so bad.  The first night he didn't stay in bed till 10:15 but slept till 7:30, last night he didn't come out of his room after 9:00 and I think was asleep by 9:30 and slept till 7:30.  We'll be changing his Med's again in a couple weeks when the pharmacy gets 15mg Focalin XR in, then we'll have to figure out what time to give him the 5mg and if the 15 works or we have to go back to 20.  He's in year round school so is out of school the first 3 weeks of Oct so we can adjust it then.

To Southernbelle, do you mean you give him a dose in the morning and around 8:00 and that works?  Or an afternoon and 8:00?

My son who is 6, never had a problem with sleep until he started taking
medication! Before he was diagnosed, he was extremely hyper during the
day, but by the time 8:00pm came around, he wanted to go to bed.
Yep...he even asked for me to tuck him in almost every night around
8:00. It was great. Since we started medication this past May, he can't
fall asleep at night.

I spoke to the pediatrician about it at our last visit, and I asked about
melatonin. He said absolutely, that we could try it. He said to use 1/3 of
the pill. So, I actually give him 1/4 of a pill, and it does seem to be
helping him fall asleep easier and stay in bed. But, the doctor never said
anything about giving him more meds in the evening. I always thought
the medication in the system is what caused the insomnia. Not the
medication wearing off. Interesting though!

Well, our pediatrician said about 1% of kids will fall asleep if given the medicine before bed, but another dr. we took him to last month (who believed in MORE medicine for everything) just automatically told us to give him more medicine before bed.  Can't really say if that worked though or not.  Because I would give it to him before bed for a couple days, then think it wasn't working and stop.  But like I said the last two nights have been great.  Maybe his body was just tired enough to sleep better the last two nights.  Don't know for sure. 

Maybe you should ask your Dr. what he thinks.  But remember, all Dr's are different.

 

Also, I don't know anything about melatonin.  How long can a child continue taking it every night?I have noticed with our son, if we don't give him an afternoon dose (5mg focalin) he has a hard time going to sleep.  We he gets it, around 8:00 he is ready to go to bed.  I guess every child is different.  Good Luck! It's a supplement you can buy at a drug store. I think they recommend not to take it for more than 4 weeks or so without taking a week off.

Dear Jennifer,

I give my son a 20xr and 5 regular of Focalin as soon as he gets up (I greet him with a smile, cup of milk in one hand and 2 pills in the other!) and then the same amount at approx. 2 pm.  We homeschool so we're flexible on times.  If I give it to him after 3:30 he will have trouble sleeping.  He goes to bed approx. 8:30 and will sleep til at least 7:30 (when his anxiety doesn't interfere).  He has always needed alot of sleep.  I try to make sure he's getting at least 10 hrs/night for him AND me.

You also asked about the melatonin.  Melatonin is our hormone that is responsible for our feeling tired at bedtime.  It is produced synthetically (not available in natural form since it'd have to come from bovine) and is very inexpensive to get in most drug stores, grocery stores, etc.  I get mine from GNC for about $3.50 for 1mg tablets.  From what I have researched and experienced, there is no known negative side effects.  I started my son on the lowest dose of .5mg and he usually always does just fine... sometimes he'll take 1mg but with Melatonin it is rare he has trouble getting to sleep.  I haen't heard that you shouldn't take it more than 4 weeks.  We've been taking it a long time with good results.  We do notice a difference though if  our bedtime is hurried and has alot of conflict.  We try to start 45 minutes before bedtime to read stories together and do quiet things so he feels settled and not overstimulated.  It makes a big difference.

PS--- Our son also gets very hyper as well at bedtime when the meds wear off. The Melatonin seems to make him sleepy enough to calm him down and be ready for quiet time. I, personally don't have a problem falling asleep on meds, however, some individuals do. A person has to be careful though. Stimulants (amphetamines) affect a persons's sleep cycle (stages of sleep) and they also reduce the amount of REM a person gets.

I try to use routine, and a lot of personal awareness (came after 20 years) to help me get to sleep at night. I still have occasional sleepless nights, but far and few between.

Thank you for telling me about your sons dosage and melatonin.  I think what I'll do is ask the pediatrician about it just to see what he says.  We have the same bedtime routine most every night, except when he has cub scouts, and we read and make sure he doesn't watch anything exciting in the way of movies, but still some nights he doesn't have a problem, and some he does.  Does the melatonin work quickly?

Also, I was wondering why you give your son the 20 mg XR AND 5 mg regular?  Does the 5 mg kick-in faster than the XR?

Ds used melatonin for  awhile to help wean him off clonidine.  When melatonin didn't seem to be doing much and I didn't want to up the dose, he switched to Gabba Calm with beneficial effects. Now he doesn't need any thing.  No one on the board ever seems to respond about the Gaba which we found effective. hmm

Ds' ADD doctor and nurse both have mentioned how a late dose of ritalin sometimes helps kids go to sleep.  I think it may help them calm down a bit to focus on going to sleep.  There is a certain amount of focus necessary to go to sleep.  lay still, close eyes, turn the mind off. 

when ds was a toddler, before he was diagnosed, I often wondered  why he never got sleepy like other kids.  I was perplesed that when he was tired, his activity level seemed to accelerate instead of slow down.  From infancy he needed a totally dark room to sleep in.  Typical techniques of rocking babies to sleep never worked with him.

Yes! It worked. I've been having insomnia problems since I started Adderall, once was even up all night. Or if I finally did doze off, I would wake up a few minutes later and start thinking about all kinds of things instead of going back to sleep.

So yesterday I took 5 mg morning, afternoon and again at 8pm. The result - the best sleep I've had for a while incl. deep sleep that I was really missing. When I woke up, I felt weak and rubbery... from my body allowing itself to relax and start recuperating. Still pretty phased out all of today. I can't wait to get another good nights sleep! So I'll be dividing my (short term Adderall)  into 3 parts from now on. 7 1/2 mg was just a tad too much for me anyway but in case I'm going to the gym or have a big gardening project, 7 1/2 mg would be okay.  I know my dosage is lower than a lot of peoples but I think I finally found my sweet spot.

Sorry I haven't checked this in awhile.  I give him 10mg of FocalinXR in the mornings, and 5mg of Focalin in the afternoon (around 4:00p.m.).  I don't give it to him before bedtime.  I just notice that when he gets that afternoon dose, he is ready to go to bed around 8:00p.m.  If the meds start wearing off, he starts getting very hyper and wil not go to sleep until around 11:00 p.m.  Hope this helps!

I have been taking 7 1/2 mg in the morning and another 7 1/2 in the afternoon but I've been having a problem with insomnia. Today I'm trying 5 mg in morning, 5 mg in afternoon and 5 mg in evening. We'll see if that helps. I'll tell ya'll about the results.

Difficulty Falling Asleep: Medication

Stimulants used to treat ADD may contribute to difficulties falling asleep in two different ways. Sometimes an individual with ADD who is responding well to stimulant medication taken during the day may have insomnia resulting from taking a dose too close to bedtime. For most persons on stimulants an interval of at least 4

to 6 hours from ingestion of the last dose of the day will allow sufficient washout of the medication for sleep onset. Yet there are some who are able to get to sleep easily within just a few hours of taking a dose and others who need an interval of 6 to 8

hours or more from time of ingestion of their last dose before they are able to fall asleep.

 

Yet sleep of a person with ADD may also be delayed due to insufficient stimulant medication later in the day. Many clinicians still encourage patients to avoid taking a dose of stimulant in mid- or late-afternoon because they fear medication-induced insomnia. Often this plan backfires because of rebound agitation or irritability occurring in late afternoon or early evening after the last dose of stimulant has worn off. Kent, Blader, Koplewicz, et al 53 have reported that administration of a late afternoon dose of methylphenidate tended to help alleviate ADD symptoms without

adversely affecting sleep. Tirosh, Sadeh,et al. reported that methylphenidate given late in the afternoon tends actually to improve sleep for many patients with ADD. Clinicians whose patients taking stimulants for ADD are having chronic difficulty falling asleep should consider both of two possibilities: the patient may need less

stimulant coverage in late afternoon or they may need an additional smaller dose in late afternoon or early evening.

I'd say the Melatonin kicks in pretty good in approx. an hour. 

I give my son both the XR and regular just because I ran out of the 5XR.  We're lucky if we get 5 hours out of the combo.   That's why he gets it 2x day.  I haven't really noticed much of a difference in whether or not the 5 regular kicks in faster.  Maybe slightly.  Possibly if the dosage was higher I'd see a difference.
Well its back to the drawing board. Taking an additional dose worked fine for getting enough sleep. But after a week it because apparent that the dose wasn't right for me. Deppression, anxiety, stress and overall just feeling like sh*t were the result. So now its back to 10 mg. a day of the amphetamine. I just picked up a bottle of Melatonin to take before bed. I hope that fixes it! I'm tired of trying to figure out the meds. I just want to feel happy and normal.
