123 magic is a good book on dicpiline forr ages 2-12, Its easy and no spanking and it starts to work in a week or so if you use it consistantly.

As for the diet changes, here are some good websites.

We started adding in supplements at age 3 (Omegas and Magnesium) and saw a huge difference.  You can read more on the Alternative and Complimentary Meds section of this board.

When my son was three we started seeing a developmental therapist we also recommended 123 Magic (works great but you have to be super consistent).

She also recommended "floorplay time" every day- a time when it's just one on one, child led play.  You do what he wants and let him have control of the direction of the play.

Of course the weekly therapysession helped too

Those things helped enough to at least preserve our sanity until he was older.

What doctor are you seeing?  If its your pediatrician, you aren't talking to a specialist.  Can you take him to a psychiatrist or neurologist that specializes in treating children?

there is a lot you could and should do before medication.  There are no meds FDA approved for kids thats young, though that rarely stops doctors from prescribing them.  But I would first try a new dcipline technique I used 123, it really worked.  You can also look into his diet.  Try cutting out sugar, then dairy. then gluten if you have too.  Add fatty acids to his diet.  Ask the doctor for a thyroid panel and a glucose test.  They might reveal something.
