**HAY EVERY ONE!!!!**1. how many dr's has he been to : 1 2. what kind of drs were they : neurologist who specializes in children 3. what were each drs dx : adhd and simple tics 4. what kind of meds has he been on (all and which ones at the same time would be helpful) :strattera, and complementary as omega3, magnesium, and vit E. 5. has there been any dramatic changes in his life (personal, school, family, friends or other) :he is doing better at school in the mornings, but I don't see much progress in concentration at home in the afternoons. Progress sometimes seems to be inconsistent 6. What do his teachers say : that he has improved in concentration, he is not agressive, impulsive or unsteady, but he still needs constant supervision from teachers so he can follow instructions better. 7. does he have 504 or a detailed IEP : ???? I don't live in the U.S 8. who is the heavy disciplinary in your home : myself (mom) 9. how do you punish/discipline him (listing all will help) : take toys away, turn off tv, time out, marble system is not interesting for him. 10. how old was he when it started :4 yrs old 11. is there any particular time his behavior is worse : afternoon, pm hours 12. have you had a complete blood work up : yes. I'm surprised you didn't include diet in your questionnaire.why? it's just a starter list of things to put in the posts not a "manditory" detail. Not some thing to dx a child. just a few things that I personally would like to know, any one can add to it any time. But you all get the main idea. ok, let's get detailed...I’m going to give you a list of questions, you don't have to answer any or all, just some food for thought for us to help you a bit more and having some more info will aid us in that. This in NO way is an attack or a "pick apart". It’s just some things that people might need to know in order to help you, help your children. And we use acronyms that you might not know and are wondering about, so they are listed below. And remember, if you have a question, just ask. The only wrong question is the one that you don’t ask... 1. how many dr's has he been to Six? 2. what kind of drs were they Two psychologists, three psychiatrists, one dyslexia expert 3. what were each drs dx Four said ADHD, two said PTSD and ADHD, two said Disorder of Written Expression, one said dyslexia (the latter being what I KNOW is true) 4. what kind of meds has he been on (all and which ones at the same time would be helpful) Strattera, Concerta, Aderall, Ritalin XR, Ritalin, Tenex (the latter is the only one that worked) 5. has there been any dramatic changes in his life (personal, school, family, friends or other) Yes, he was an older foster child when he moved in with us. He went from living on the streets to living here. HUGE change. His bio family is not in the picture, at all. 6. What do his teachers say Now, they love him. Took off Strattera and Concerta and switched to Ritalin LA. Currently on 40 mg Ritalin LA, 15 mg methlyn in afternoon, melatonin at night. We may be upping the dose tomorrow. Teacher says she isn't paying attention in afternoon. :( 5. has there been any dramatic changes in his life (personal, school, family, friends or other) No 6. What do his teachers say (last year it was "doesn't work to potential, fails to turn in work, doesn't pay attention..." I have conferences next Thurs...I will let you know) 7. does he have 504 or a detailed IEP (not yet, thinking of working one up for middle school next year if need be) 8. who is the heavy disciplinary in your home (I do most of the day to day stuff, but call out my hubby for the big guns) 9. how do you punish/discipline him (listing all will help) (Earlier bedtimes, take away playstation, writing sentences--he HATED writing til recently, extra chores) 10. how old was he when it started (as an infant he had a 'witching hour' almost every night at around midnight where nothing would calm him, he could not even be in a car for more than 5 minutes w/o crying, if I didn't stop and murse him, he would cry til he puked...had biting issues in daycare/preschool, but I worked there so maybe that is why he never got booted out) 11. is there any particular time his behavior is worse (not really) 12. have you had a complete blood work up (no, and I am wondering why??) He really is just having problems in focusing and getting organized, and this year his grades have done a real about face...he still had the 'not turning in all of his homework comment' on the back of his grade card, but when I went to the website for the school there were no blanks for grades...a few "late/incompletes" so I will quiz the teachers at conferences to see how big a problem this is. He reports to all who ask that he feels tons better with his meds. He called it his "happy go lucky" pill the other morning and his self confidence is through the roof as opposed to pre med "I am too dumb to ever get it mom" 1. how many dr's has he been to: 5 2. what kind of drs were they: pediatrician referred us to the therapist, therapist led us to the psychiatrist, went to a diff psychiatrist for a second opinioin, therapist also suggested taking him to a special hospital which we did. 3. what were each drs dx: severe adhd/mild odd 4. what kind of meds has he been on (all and which ones at the same time would be helpful): risperdal, strattera, focalin, adderall, adderall xr, concerta. risperdal has been a constant, we tried all the above and then stayed with straterra/risperdal combo and just recently added concerta, which might i add is having negative effects. 5. has there been any dramatic changes in his life (personal, school, family, friends or other): me and his father separated when he was 2 but i don't think he hardly noticed and we still make time to have "family days". he has a stable family background. my sister helps me raise him. his only complaint seems to be that mommy is always working (i work 3 jobs) and he hardly see's me. which i agree with, but there's hardly much i can do about that. we go on a "date" every week and i take him to karate on wednesdays. 6. What do his teachers say: he attends a special school and although he's improved he still has trouble focusing on schoolwork and needs a lot of one on one. 7. does he have 504 or a detailed IEP: detailed iep/ibp 8. who is the heavy disciplinary in your home: my sister. 9. how do you punish/discipline him (listing all will help): time outs, if he doesn't get ready for bed fast enough at night he looses minutes off his story time, 10. how old was he when it started: i noticed that he was different as an infant, contantly crying, as a toddler he was too hyper. i thought at first it was first time mom jitters and my sister agreed that i was being paranoid but when babysitters started telling me he was different that's when i and finally her started to notice more. we started therapy before he even entered kindergarten because he'd been kicked out of 3 daycares. 11. is there any particular time his behavior is worse: after coming back from his grandparents house. we tend to stick by a behavior plan but they don't, choosing instead to spoil thier only grandchild and even rewarding him for his bad behavior just to get him to calm down. we're normally stuck "re-training" him for a day whenever he goes to spend the night. 12. have you had a complete blood work up: I honestly am drawing a blank but i believe this was done when we took him to the hospital. ok, let's get detailed...I’m going to give you a list of questions, you don't have to answer any or all, just some food for thought for us to help you a bit more and having some more info will aid us in that. 1. how many dr's has he been to - 10 2. what kind of drs were they - social workers, Md's Phyiatricts, Psychologists 3. what were each drs dx - ADHD, ODD 4. what kind of meds has he been on (all and which ones at the same time would be helpful) - Concerta, Adderall XR, (Strattera, Risperdal, Wellbutrin, Gaunfacine) He is taking these now 5. has there been any dramatic changes in his life (personal, school, family, friends or other) - Not really 6. What do his teachers say - he is not paying attention - he is very disorganised - he argues - he insists on doing things his own way - he loses his work - he spaces out - he doesn't try very hard - he has temper tantrums - he hates writing - I could go on forever 7. does he have 504 or a detailed IEP - a detailed IEP 8. who is the heavy disciplinary in your home - I am, but he worries about DH more - boy thing? 9. how do you punish/discipline him (listing all will help) - reduction in allowance, extra chores, time out. 10. how old was he when it started - as an infant - he would scream and cry for hours and NOTHING could console him 11. is there any particular time his behavior is worse - after winter break he is always worse.
