You might discuss the option of guanfacine (Tenex) with the doc. It is given to reduce tics and has effect for some ADHD symptoms as well (bonus).

It might work instead of the stimulant or with it. Here is some info:

vickie39029.4106365741Sometimes puberty seems to bring changes that change the way the body responds (or doesn't) to meds. I have read that meds seem not to work premenstrally for some. Also, has there been any increase in stress for your daughter? This can also decrease how the meds work. You should explore these issues with the doctor and decide if it is time for a med change.

Hi tsliber, my son who is 12  has been on concerta for about six years now, if his weight changes at all it starts not working right. Last year in school it stopped working, however the teachers didn't inform until the year was just about over, he was on 72 then. He now takes 90 mg, and is doing great on it.

My son has been on every combination/dosage of all available meds for
the past 5 1/2 yrs. He is now 10 and on Concerta 27, Strattera 40 since
neither helped by themselves. He had been on Concerta 54, then 36, but
we asked the dr to decrease it more and more because of facial tics and
other side effects. As most on this board knows (except the lucky ones),
there are always difficult periods every day of his life as the meds are
released into his bloodstream or he's coming off it. Currently, he's
making loud snorting grunting noises mostly in the afternoon, although
teachers and others hear them throughout the day. We're going back for
further adjustment of the meds although he's been on this combination
for a year or more. Anyone experienced these kinds of 'tics'?? Clearly
there's a problem with the meds although his overall behavior and
schoolwork are better than before he took this combination. We've
adjusted meds so many times and there is such a chaotic period of
transition with each change, I'm gun shy. I'd like the dr to consider
Daytrana but since he also needs the Strattera, who knows? BTW, when
other family members hear these noises they think we're insane for
keeping him on meds, but you've all heard that, right!my daughter (she is 12) has been taking concerta for about 4 years.  the dose started at 18mg and is now at 72mg.  she just started the 72mg a week ago because the 54mg wasnt effective anymore.  but now it seems to not be working at all.  is this common?  should we explore other medications?  she has been on ritalin and adderal before the concerta...
