There is a thread where the use of nasonex on the skin prior to the patch was suggested by the child's doctor:

You might want to discuss this option with your child's doctor.
My son has been on Daytrana for 4 months now with just the little redness when on the patch and after taking it off. It does stay a little red the next day but we have not had any real problems before now. I don't know if its because of winter dry skin or what but his poor bottom is rashy, very scaly and itches like crazy. I have been slathering him in lotion which he says burns even though its fragrance free and hypo allergenic. Any other suggestions or can we put the patch on the leg or arm? I don't think he would be having an allergic reaction after being on it for 4 months but you never know.. Anyone else?I don't have experience with Daytrana, but I do have sensitive skin and have experienced burning with lotion even when it's fragrance free and hypoallergenic. I find that Cetaphil CREAM works great and it doesn't irritate in the least! You can pick it up at most grocery stores, Targets/Kmart/Walmart, or drug stores. It may or may not help with your son's skin, but if you ask me, it's worth a try!
