My son is seven years old and in the first grade. He has tried a few different medications. One was a patch that didn't work. The other two he just adpated to which was focalin and adderall. It seems that the adderall works at school but when he gets home its out of his system. He goes completely crazy. I 'm not sure if the medication is the problem when he gets home because he has a two year old and newborn sisters. If its just a sibling problem how do i deal with it? He always wants to play wrestle with the two year old and keeps touching the newborn all the time when i am dealing with the newborn. Then he plays and takes the toys away from the two year old. What should i do? Plus, when i tell him to do something like pick up his room or be quiet. Its takes me telling him about five times. I'm just not sure what to do!

When was he diagnosed? By a pediatrician or a Psychiatrist? what dosages of the meds have you tried, and for how long on each one?If you are not seeing a psychiatrist, you may want to consider one. They are more experienced with med managment and tayloring the meds to your child's needs.
Your son may be having rebound as the med wears off, and the meds are useful for social issues as well as the achedemic issues. If there is something working for school, you might look into getting a second dose before the first one wears off, or a lower dose of a short acting form to ease rebound.
Look into a positive reinforcement behavior plan like the marble program by ogram (top thread of this board) to help reinforce the behaviors you want to see.
