18mg is the lowest dose of Concerta. When my son was on it, neither I nor the teachers saw a huge difference. My son is the inattentive type...he ended up on 54mg and did quite well on it, except for appetite issues, for almost a year. We switched to ritalin LA and we both like it much better because it does not last as long, and he is hungry by dinner time. If he was the hyperactive type...I would probably want it to last into the evening.
Can I ask what GAD is? I am clueless, but it may just be I have not heard of it....
The doctor should titrate the dose upwards until you see lasting results.

If you have the correct dose you should see changes within a hour of taking the medication. Let us know how he is doing.
