So my doc initially prescribed that I take IR twice a day.  At first, with lots of side effects, i was afraid to take it twice a day, and began taking it once a day. 

Now, the side effects from my one-a-day have worn off, and about 12 hours after the morning dose, I become absolutely worn out exhausted and can barely keep my eyes open (but can't exactly fall asleep either)...    Does that sound typical?  

I imagine I need to be taking it twice a day now?  When do you folks normally space out your dosages?

 

for me the IR lasts 4 hours (why I prefer the XR).  If 24 hours pass without a dose, it's hard for me to stay awake.... this sleepiness lasts about a day on the rare occasions that I take a "holiday".

 

Good question!  I can only speak for myself... if I recall correctly, I was told to start out the first week taking on 10 mg. in the a.m., another 10 mg. around mid-day, and to monitor how I felt.  I was told that if 10 mg. was having little effect, to try taking 20 mg. in the a.m., which I did after about 4 or 5 days.  I went back 2 weeks later and was told I could up my afternoon dose to 20 mg., and that's the dosage I've stayed with: 40 mg./day of Adderall, split up in two doses.  Based on what I've read from others messages, I think my doc was less attentive (imagine me call someone less attentive!!) about working up my "optimal" dose--in fact, he flat out said he wasn't interested in finding an optimal dose!

how do doctors begin dosage once someone's diagnosed?  I guess you start low, like 10mg, then work your way up?  How long does this take?

I started out taking 10mg twice a day. But I would want to fall asleep about an hour after the dosage (even though I couldnt sleep). I was bumped up to 15mg, 20mg, then finally 30mg. When I first started the 30mg, I could only take one a day because I felt like garbage when taking two, my side-effects were so strong.

But after time, I found that I was able to take my second dose about 6-8 hours after the first, and everything seems to be fine. The second dose gives me a little kick of energy at first, but then I mellow out. Also, I dont notice any real side affects except for decreased appetite and dry mouth.

I guess the best method is to slowly experiment with what the doctor gives you. See what works and what doesnt, but do it carefully. If you cant find a method that works, its probably time to up the dose.
