I agree with Vickie. Keep a journal, especially for when you increase. If it is causing you increased anxiety, that may increase with a higher dose. Or if you just are not at a high enough dose it will get better once you increase. Keep in mind just because Concerta is methylphedinate it is in a different delivery system than Ritlain and will not be the same for you. Every med is metabolized different and has a different effect on every person. Even the same med in different delivery form. My daughter could not handle regular Ritilan or Concerta, but did ok on Ritilan LA. There are many options out there just keep trying and write it all down then discuss with your doctor. Again as Vickie stated you can also add to the Concerta to help with rebound if that is what it is. Good luck and hang in. Don't suffer with unpleasant  side effects.

It not lasting long enough can be due to either the dose being not quite high enough or because you metabolize it a little quicker that others. What you are feeling may be a little rebound or an increase in anxiety triggered by the stimulant. This may go away as you adjust to the med or it may not. If it does not go away, you may need to have a low dose of a short acting form of the med to ease this period. It would increase the effective period as well. Usually these issues can be handled. Keep your doc informed on the anxiety. You might want to keep a diary to help sort out the treatment and make adjustments to meet your individual needs.

I have found some very useful information on this forum.  I want to see if anyone else has had the same experience.  I was diagnosed with ADD about 12 years ago and used Ritalin successfully for quite a while.  I have been on no meds for the past 5 years due to pregnancy and breastfeeding.  I have been having a lot of trouble coping with my ADD without meds and last week started on 36 mgs of Concerta.  I am supposed to do 7 days on the 36 then up to 54.  My dr. said it should last about 12 hours.  I am taking it between 7 and 8 AM and having wonderful results.  The problem is that it seems to be wearing off between 3 and 4 pm and when it does I experience very unpleasant feelings of anxiety, nervousness and feeling like a tightness in my chest.  Is there anything that can be done to help this?  Is this a normal feeling?  I am not normally an anxious person and these feelings almost make it seem like the benefits of the meds during the day are not worth it.  I am also wondering if this might be just a temporary side effect.  I dont remember feeling this when I was on meds before.  Any input would be appreciated!
