Prior Authorization (PA) requires a drug’s prescribed use to be evaluated against a predetermined set of criteria before the prescription will be covered.
Certain drugs require prior approval before coverage is provided. To initiate a PA evaluation, physicians will need to submit additional medical information and must call Caremark at 1-888-413-2723. Pharmacists and physicians will review the medical information to determine if you meet the criteria established for coverage of these medications. You will be informed in writing as to whether your request for coverage has been approved or denied. Drugs requiring Prior Authorization include, but are not limited to: Growth Hormones, Retin-A for individuals age 26 and older, Desoxyn, Dexedrine, and Adderall.

yargabarga39276.788900463That's odd, we use Caremark and I have never had to get prior authorization for anything.Seems I need prior authorization from Caremark before I can get my Adderall XR. Anyone else have experience with this? I heard that you can get denied and your doctor has to spend a bunch of time giving reasons why I would need it blah blah. At the pharmacy the rep made it seem like it was pretty standard though and that everything should be okay by Monday.
