Do you think the meds are at the right level? I agree with the adding of guanfacine. My son has been on concerta and is 10. He takes 90mgs along with 1 mg of guanfacine/tenex.
I would discuss the upping the concerta with the prescribing doctor, as well as the addition of guanfacine. This will help control any impulsive behavior. I would do this before I change meds all together.
He just sounds like an adhd not medicated to me. (My son did the poke with a pencil when he was in first grade - that lead to the meds beginning!)
He is also probably beginning puberty at almost 12, remember that will change meds all together!

Sounds like a visit to the Psych is needed, and maybe a med change.My son is 11 and 1/2 and in the 6th grade. Here is whats going on:
Throws pens and students
Jabbed a student with a mechanical pencil when (provoked supposedly)
Pokes people
Asks the same question over and over, like can I do this, or buy this
This is after he has taken is 54 mg. of concerta a day.
Before meds in the morning:
Stomps on the floor
Bangs on the kitchen table
Looks for things to do to irriate family
Copies people in family.
This is just a brief list, he also NEVER takes responsiblity for his action, always has an excuse or reason to blame someone else.

I assume you have had him dx by a doctor b/c you have him on Concerta. There are many different symptoms to ADHD and no child alike has the same ones. Also, there are things that can mimic ADHD symptoms like food allergies. Also, some meds don't work you might have to try another. I think Concerta is what a lot doctors try first. Sounds like an impulse issue and I have my son on Tenex for that but that is the only thing he is on along with supplements.
