Concerta rebate | ADHD Information

The rebate form came in the mail. I get about 1 a year from McNeil labs. Its a really big glossy flyer.  I tried checking theri website and could not find anything there. Thanks for your help.Where did you get that? I had a card for Concerta that came in a little box, explaining all about the drug. Included was the card to get 50% off the first prescription, and 25% off the next two. I'll do some looking around, but if it helps, there is a number (no address) listed on the card. It reads Pharmacist Only (and I know you're not but it might not hurt to call) 800-422-5604.  The issuing company is Therapy First, for McNeil Pediatrics.  I suppose you could call that number if you see Therapy First somewhere on the rebate form. I'll check around and see what I can find because it can't hurt, probably, to get one of those rebates for myself
I received a Concerta rebate form, ripped off the return part, but there was no address listed on that part. Does anyone have the address where I can send in the claim form?
