Advising the School

My son was just diagnosed with ADHD this past weekend.   Although we were fairly certain he had some form of this it still is so awful to actually her the words from the doctor that he does in fact have this.

I have to advise the school board (special education department)that we are requesting that he be evaluated and considered to be placed in the 504 category.

Any advise on composing the letter and what I should ask/not ask at this juncture.
