Some kids do better on multiple doses. My son was on Focalin XR 5 mg (9 years old) and sometimemes upping the dose can help as well. Melatonin is reallly good for sleep and is all natural.Why not try 10 mg regular after school?  That will probably be out of her system by bedtime.  If not, you'll know it the first day.  We've never had any problems with methylphenidate at 4 p.m.  You could try just giving her two pills and see what happens.

Is the doctor a psychiatrist?  If so, I'm surprised.  I think that a lot of people do a dose after school.
My 6 year old daughter was diagnosed with ADHD 3 months ago after many years of behavior problems. I was very thankful to finally have an answer to what was going on and was hopeful to making things better.
She has been taking Focalin XR 10mg in the morning and 5mg regular focalin after school for 2 months. While the 10mg in the morning seems to be working the afternoon dose is not. She is very mean to her little brother. She damages things, hits, kicks, and screams alot. Geting homework done is a nightmare! Her doctor says he can't up the afternoon dose because she might not sleep at night. He wants her to take another 10mg XR during school to get her through the evening. I don't like the idea of her having to take medicine at school. Does anyone know of any other options?it may be that the doc is thinking she does better on the long acting and that is why the thought of adding another XR to her day. I think it's worth trying the 10 after school for a day, anyone can handle one night of going to bed late. She may just not respond as succesfully to the short acting stim. If that doesnt work or she is up too late, you can discuss adding the XR at school or trying something else.
