I think you hit the nail on the head with "Duh." My son's 3rd grade teacher did that all the time- "he needs to pay attention to his work" was often written on the top of his papers. It made me want to scream.
How much is the impulsivity effecting her work/behavior? It could be that a med adjustment would help, but if she has trouble with side effects it may not be worth trying a higher dose if the problems aren't too bad.
You should definetely talk to the teacher. They are human and sometimes forget things (like maybe the symptoms of their students' disorders), but she may not know much about adhd either, in which case she could be thinking that the meds have everything under control of she's not hyper at school.
I don't know of a cure for sloppy work- if you find one let me know.
