I agree with Jessica that he seems pretty complex and there are a lot of things that are potentially going on.  You won't know until he's a few years older which of those traits stay and which dissipate.

If I hadn't been so stuck on the idea that I was somehow a bad parent when my son was 3, I would have taken him to an OT and myself to a support group.  If you get a good OT, they will be able to give you guidance (and will also know the good doctors to go to for a full evaluation when you are ready to do that.)  I wish I would have found this website back then, so at least you have that advantage . 

Easily over-stimulated and hard to settle down

Hard to fall asleep (45 min, sometimes 1.5 hours or longer)

Aggressive towards other kids (and sometimes adults), hit, push, etc, even babies

Prefers to play at home or alone/small groups

Hates crowded playgrounds and withdraws

Difficulty with transition

Mood swings

Intense and long tantrums, used to bang head

Distracted during circle time and teases others by nudging, kicking, etc.

Pushes kids that get close to him

Doesn’t follow instructions

Refuses to try challenging tasks

Always moving (during meals, spins)

Only likes hugs from parents

Quick temper, easily frustrated

Very impulsive

Destructive
