concerta 36mg | ADHD Information

My son (9 y/o) was diagnosed back in Dec - the Dr upped his dosage to 36mg and he was doing okay for about two months - the past two weeks we have noticed a real regression of his behavior - taking him 5 hrs to clean his room, talking back, getting angry when not everything goes his way, school behavior inattention when in group activities etc... Can he already have built up a tolerance to this dosage?  I am extremely frustrated and any idea are welcomed on what to do next and I value everyones helps and comments on this site because I know that you are going through or have gone through similar situations... (I did put a call into the physician to ask her opinion as well)

my son started concerta 18mg in early feb.  in 4 weeks he reverted back to non-med state, so i upped to 27mg.  within 2.5 weeks i noticed symptoms returning and by 3.5 weeks he seemed uncovered again.  i just started 36mg yesterday.

i know what you mean about tolerance.  i'm not sure if what you and i are experiencing with this is just the normal part of determining the right dose or if our children are developing tolerances to the meds?

 

We are having the same issue. 7.5 year old  DS was on Concerta 18mg for about 9 months and then we saw some of the same issues pop up again.  Started 27 mg and for about 4-5 weeks it was awesome, then the same issues have started again. 

I am really curious as well to why this happens, is the tolerance buildup this quick?  It is really a bummer when they are doing so well and within a couple of days it goes back.  Not only a bummer for us but more importantly them!!!!!

 

I talked with his Dr and she had put him on Ritalin 20mg bumper dose that he will receive at noon.  It was something we wanted to avoid doing - giving him meds during school time because kids already comment on him and call him a freak, but she said to try this first so we are going to see if this will help him control his behavior more since we were seeing the mornings going by ok but afternoons were much much worse.

i know what you mean - i wish i had the answers - don't we all - anyhoo - he seems to be doing well with the bumper at noon but of course now his teachers says that he is doing well in the afternoon but he is seeing more inattention in the mornings - so i am at a loss - am going to talk with physician next week about it and see what she says.

 

I don't think it is so much tolerance as they haven't reached the optimum level for them.  It is very common to have to tweak the meds until they reach that point.When you go from nothing to a low dose, you will see an immediate improvement.  It is one extreme to the next.  A little bit more and more of the symptoms fall by the wayside.  When you reach the max. effective dose you shouldn't have to change for a long time[or puberty, for some]

do you know why symptoms improve and then return?  i thought this was going to be a situation of constant improvement as the doses increased until the side effects outweighed the good.  and when symptoms return, how long should we give until determining if it's just a hiccup in the process or a definite indication of a problem?  for me, i gave it about 4 days on fairly consistent problems which seemed to suddenly re-appear.
