Definitely call the doctor. This sounds like he needs a change in meds. Medication is so individualized. You can have two siblings that react completely differently to the same med.
This is another case to support going to a ADHD specialist, pediatricians do not have the expertise with meds and often are baffled when the standard treatment does not work. An ADHD specialist has the pharmacalogical (spelling??) background to find the right med, they are willing to prescribe stuff a Pediatrican wouldn't.
Last fall I had to visit the Ped for a refill when my son's psychiatrist was out of the country. The peds was hesitant to refill his lamictal (mood stablizer). I explained the purpose and he finally did. Lamictal is normally prescribed for eplipsy but off label use includes mood stabilzation. This visit also reinforced my opinion on being well versed on his meds and why he is taking them. Knowledge is power. I feel the doctors treat you differrently when you are informed and knowledgable, more like a partner than just the parent of a patient.
i just wanted to tell u that after 4 weeks on meds it seemed they were not as effective for my son. he started on concerta 18mg and then went to adderall 10mg which did nothing then went to 27 mg of concerta. concerta seemed like a dream come true the first week or so and now it doesnt help very much. im thinking this is just not the right med for him. tomorow we are going to the dr and we are going to try vyvanse if they will let us and insurance covers it. im thinking maybe the same thing with your son. its probably not the right one if its making him feel bad. they shouldnt change his personality. when my son started taking concerta he was the same little boy only he could focus more and sit still for more than 2 mintues at a time. he stopped bouncing off the walls. but he was still happy and making jokes. i really hope u can find the right med and dose for him as well as i hope i can find the right one for mine. just wanted to let u know that u are not alone. we thought we were one of the lucky few that get it right the 1st time but the search is back on! good luck!keep us updated.

You need to call the doctor and tell him. It is not at all unusal for it to take 4 weeks to notice these effects. Remember that it can take up to 4 weeks for some meds to show a change. It depends on alot of factors including the child's age and weight. He probably need a different kind of med. Some kids do better on Focalin versus Ritalin. You need to be sure to avoid any caffeine with Metadate. Metadate is a form of Ritalin. My son can not take any Ritalin specifically any methylphenidate because it makes him more aggressive and hyper. He can take the Focalin which is dexmethylphenidate. Go figure? I am told that he has to do with brain transmitters, dopamine and norepinephrine.No med should make a child/person change. Call the doctor ASAP and find out what is going on!!
They may have rebound but that is toward the end of the day, like afternoon. usually their blood sugar is low, and they need food, but the med supress their appetites. I gave my son a bar of chocolate and it worked like a charm for him!
My son also had trouble with strattera and he told us he wanted to kill himself, once. we stopped the med immediately and never went back to it!! Adderall XR made him aggitated, angry, horrible.
Everyone is different when it comes to these meds. Some have success with what one has failure.
DS is on concerta and guanfacine(tenex) for the past 5 years and doing well!!
