Need advise | ADHD Information

I've been raising my grandchildern for four years, my 5 y/o was just diagnosed with adhd and adjustment disorder with disturbance of emotions and conduct.(behavior) . My other grandchild has adhd and will also be getting a full behavioral evaluation, we are still looking for the wright medication for her because of her medical issues. He was put on Tenex 1mg in the AM and he didn't do so well , called the Dr's and had it decreased by 1/2 , seems to be Ok but, around 3:00 he's back to himself. Today I gave him the other half, I'll call his Dr. tomorrow. Does anyone know how long it takes for this drug full affects too kick in,  this is his 1st medication. This is all new to me and it's been a long journey.Started him on 1/2 tab in am and noticed @ 3pm the meds had no affect anymore , so I've been giving him the other 1/2. His teacher says he's doing really well at school except he's getting a little tired. We are 3wks into his medication and I find he still has times when he can't controll his impulsivness should I Contact the Dr. or wait a little longer. He also has a fear of load noises like thunder and the fire drill bell at shcool, is this a common sympton of ADHD He will ask my every day if there is going to be a drill or if it's going to thunderFrom my understanding, any ADHD medication doesn't last all day. It will last about 8 hours or so, give or take. The purpose is that they are able to 'properly' function during the day when they may have school, daycare, etc. My son takes his medication in the morning, and about 3pm, he's back to his old self, which we try to manage at home where we know how he behaves and there's no danger of being kicked out of something important. As Mandie said, most adhd meds only last up to 8 hours or so. Each one
has their own time release options. Most take two weeks to reach their
full potential. My son in on concerta at the moment and it last 10 to 12
hours. Meds are a hit, miss kind of thing. You try one and if it doesn't
work, move on to the next. Keep your dr. informed of how the meds are
working, if there are any side effects, and when it is wearing off so you
can discuss your options together.

I chose to have my son take something that works almost all day, or
before this med, we gave him metadate twice daily, until it stopped
working. While on his meds he is extrememly well behaved and like a
totally different child. I saw him being without meds for a longer period
being harder on him cause it seems he can't control his actions.

Reward systems are great for adhd kids. They respond so much better
than they would to say, punishments like timeout and things.   Or at
least that is what I have found to be so with my son.
