Sounds like ODD (defiance) is creeping its way into the mix with the ADHD. Having a name can help you look for lots of info.
A positive reinforcement plan can help (rather than just punishment). Take a look at Obrams Marble system stidkied on the parents board.
Med adjustment or agumentation may be in order (there is room for help here), so talk to the doc. What kind of doc does the med management (ped or psych)? I only ask because psychiatrists have more experience with med management and that experince can be essential if treatment needs to take into acount things like ODD, anxiety, other issues.
My youngest used to respond in a similar manner to hide how hurt she was. This can be hard to sort out.

I apologize in advance if this is all over the place. I'm not even sure where to start.
My DD is 6. Has AD/HD combined type. She's on 10 mg Methylin ER. She is repeating Kindergarten. She has trouble reading because she has some visual perception issues. I'm not sure if any of this information will help, but just throwing it out there.
The past few weeks, Kalynn has been getting into trouble at school. She's very defiant. For example, yesterday, she dumped toys out and refused to clean it. She was put in time out and not allowed to play at recess. Today, same thing. She refused to clean up her mess. While sitting in time out, one of the teachers was sympathetic and said something like "It's sad to miss recess", and my daughter responded "No it isn't. It's funny!" and proceeded to laugh. I'm not exactly sure what else happened, but she was sent to the office.
I am not sure what is going on with her. I don't even know where to start, or how to handle this..
Any input would greatly be appreciated..
I feel your pain. My son is 11 and has ADHD and ODD. We've been dealing
with this since he was 6 years old, I would also suggest medication
management with a psychiatrist.

I'm new here, but thought I would chime in and say hello, and maybe offer some advice (seems like you've gotten some good ideas so far):
1. Definitely schedule a conference with the teacher if you have not done so already. Take a list of your concerns with you. See if you can also include the resource/special ed (whatever it's called in your state or territory) teacher as well. Talk about strategies for handling the behavior issues. I know it doesn't work for everyone, but we've found that a very 'matter of fact' action/consequence or reward method works for us.
2. Our doctor asks a lot about defiance on our regular visits to review meds--definitely bring it up on your next visit (or schedule one). I know we've had to deal with defiance periodically as the medication level was adjusted.
3. Going back to #1--if you don't feel you receive satisfactory answers/help, don't be afraid to step it up--to the lead teacher or asst. principal or headmaster--depending on the school structure. (I am speaking as a teacher--and as someone whose son had a *terrible* teacher last year and an *awesome* teacher this year--the difference this can make is absolutely amazing).
4. Visual perception: I'm not sure I understand--do you think she needs glasses or something like that? Luckily, that's pretty easy to check with an optometrist.
I am sorry this is happening--although I am new, I think I'm making a correct statement when I say I bet a lot of us here feel your pain and know how it is to be frustrated when all you want is the best for your kid.

You mention that your child has visual perception issues which affect her reading ability. This could be considered a learning disability that is separate and apart from ADHD and your dd could be acting out behaviorally due to low self esteem issues. A diagnosis of ODD is a very difficult diagnosis to make , even for a trained clinician and such assumptions are not helpful nor is behavior modification when no one understands why the child is acting out. The inability to grasp reading concepts and being held back which doesn't help the child make significant gains academically for most can be frustrating and embarrassing for the child. I think your child is masking her fears by pretending not to care and laughing about it.
Psychiatrists are great for med management but your dd may need to receive talk therapy from a counselor or a psychologist to help her deal with issues of low self esteem. Low self esteem issues are not uncommon for children with ADHD and learning disabilities. While a good behavior plan is one of many excellent tools that are needed, she cant be disciplined out of feeling bad about herself and I think she is crying out for help, not purposely trying to be defiant. Again, even a professional has a hard time identifying ODD because children with ADHD act out on impulse, not intentional defiance and a learning disabled child acts out because sometimes they have a problem verbally expressing their feelings constructively or don't feel comfortable doing so. I would reevaluate her med situation with a trained professional and also look into talk therapy A therapist will give your dd tools to cope with her feelings and also establish a good behavior modifcation plan for you to follow which is suited to your dd's needs.
Given her temperment, I would switch to a child psychiatrist for medication management.
