Has anyone been asked to fill out one of these on their child? The "behavioral specialist" sent one home for me to fill out on my 3 1/2 year old daughter. i am hesitant to fill it out as we are working with a developmental ped who diagnosed her as ADHD last June. She just started taking Adderall (10 mg) once a day and we have an appt set up with a ped psychiatrist next week.
Any advice?
If the BASC is the long complex form then yes, I've filled it out. When my son was evaluated by the school board we filled it out. Since then the doc and teachers have also had us fill out much shorter forms. I think the BASC is a questionaire that is used fairly often to help diagnose behavior issues including ADHD.Describe the questionaire? I've filled out a lot but I can't remember what BASC is? Is it really long or fairly short?
I'm actually still waiting for it. The teacher was supposed to send it yesterday but I haven't seen it yet. I'll update you when I get it!

Oh yea, definately completed that one. WHen my son was almost 6, one of his doctors gave me this to fill out. Lengthy, complex, and really made us think about his behavior. I would imagine it is used as as tool to dx adhd (since there are no real tests). I rec'd additional copies for my son's teachers to complete as well. Surprised to hear that your daughter is medicated at 3 1/2. Even though I was convinced my son was adhd (he was adopted at birth, all his birthsibs are medicated for adhd, his birthmother warned me before delivery what to expect) I could not get a diagnosis for adhd until 6. So at least you are on the right path without wasting any time! My son (now 10) has responded very well to focalin XR and is doing excellent academically and socially. We see a pedicatricn neurologist every 6 months to monitor and work with a great behavior doctor. Plus he had three years of working with an occupational therapist for his sensory issues.
