Sleepiness in the morning with Vyvanse? | ADHD Information
Hi, I am a 33 year old male student who has been recently been diagnosed with ADHD. Once I found out about ADHD, all of the sudden many things in my past life made a lot more sense. The constantly being told I never lived up to my potential by teachers, the falling asleep in front of my text book (when little) no matter how hard I tried not to. I always thought I was just terribly lazy-- smart enough I guess, but with absolutely no motivation. I would see others turn in their homework consistantly and always envy them. I never could understand why I was never able to bring myself to do the same... In short, I thought I was flawed.
Now I am beginning to really have hope that there might be a light at the end of the tunnel. I think stability and contentment might actually be attainable for me after all, not just faking it, and hiding my issues like I have learned to do all my life.
Anyway, on to my question. My p-doc first prescribed 18mg Concerta for me. It seemed to help a little I guess, but was very subtle to be sure. A month later he bumped my dose to 36mg and this seemed to be a little more helpful, but it didn't seem like it was completely effective yet-- maybe at around 50% effective. So , naturally, when I told him this, he bumped the dose once again, this time to 54mg. I was very hopeful about this dose as I believed that the effectiveness would continue to scale upwards, but sadly this was not the case. It seemed to do the exact opposite. Instead of an increase of my focus, I was literally falling asleep all day! I was utterly baffled by this. I went back to my p-doc one week later and asked him about this. He said that this was normal, that it was supposed to calm me down some. I should note that while I was very hyperactive as a child and even into adolescence (people always asked me if I was on speed-- I wasn't, lol)-- I am not really hyperactive as much anymore, I just have a terrible time maintaining focus for more than ten minutes. I told him that I cannot be falling asleep all day as I have work to do.
So he ended up prescribing Vyvanse at 30mg. I had read about all of the common ADHD medications, so I was familliar with this one and thought it was the a good choice to try next. I had and still have high hope for this medication, but I have one problem so far: it makes me fall asleep in the morning!. I wake up at around 5:45 AM and take the Vyvanse and then go back to sleep for about 45 minutes. When I wake, I feel a very slight stimulation, which lasts about an hour, then I begin to yawn a lot and find it hard to keep my eyes open. I get so tired that I sit down in my recliner and end up napping for about an hour. After that I feel better, and then at around 10:30 AM, I feel more pronounced effects of the Vyvanse-- a very mild stimulation. It does increase my focus some, though I feel as though a higher dose might be more beneficial. While the effects take a while to kick in, they do last all the way until like 8 PM for me.
The main problem is the falling asleep in the morning. To be fair, I should mention that this is only the second day of my Vyvanse treatment so far. I also take 20mg of Lexapro and have for about a year, and 50mg of trazadone with 125mg of otc diphenhydramine (Benadryl) for sleep (as suggested by the doctor because it is effective for me).
Could one of these other meds be messing with the Vyvanse? Does this med take a while to build up or stabilize maybe? I have googled quite a bit and found very few references to anyone falling asleep at first with Vyvanse, just once or twice in passing, and never with a followup answer. Has anyone else experienced anything like this? I am wondering if an increase in the Vyvanse dose might help with the morning sleepiness as I have read that 30mg is a small dose. Or perhaps a IR stim might be warranted for the morning?
