Lost teacher questionaire | ADHD Information

The Connors and Vanderbilt questionnaires are common ones.  However, I don't think they are available for free, so I don't know if you'll be able to find them online.  At a worst-case scenario, you can get the form from the doctor January 3.Help, I lost the teacher questionaire that my dd's doctor uses to measure her response to the medicene. I am not sure if the doctor's office will be open next week and her appt is Jan. 6. I believe the form title starts with a N and has one or two tt's in it. Hoping someone knows what I am talking about and can direct me to a link for the form. Thanks in advance.
