 I have problems with trust and dependability in relationships.

Any advise for following through, and remembering important things?

I'm not really sure I understand. Would you please help me out? Do you mean you are not trustworthy or can be counted on or do you mean you find people to be in a relationship with who are not to be trusted or counted on?
